The ITaP Customer Service Center is promoting the use of an alternative way to send in request for assistance to the service desk. It is the CSC Request Assistance Form. This form may be used by any of our customers.

Using the web form gives the customer service representative a head start in resolving your request. Since on the form you will be prompted to enter key information such as your name, alias and affiliation with the university, that information will be readily available to be entered into our request system.

Under the Incident Information, heading, you are asked to enter a brief descriptive subject and then enter a complete description in the larger text box that follows. Please provide as much information as possible in the description of the issue. Include details such as the name and version of program being used, computer location, and any error messages received. As a general rule, the more detailed information, the better.

When you submit the web form, the information is sent via email to the ITaP Customer Service Center. This is done without using an email client program, making it possible to send in a request assistance form, from any computer with Internet access.
