New Mainframe Access/Form 182 Process:
  1. New staff need initial mainframe accounts (ACIDs)
  2. Departmental Requester completes the Form 182 on-line.
  3. They print, complete the form, and sign. They also obtain the employee’s (new user’s) signature and the Department Head’s (or designee) signature.
  4. Unsigned Form 182’s can be submitted, as long as the Requester agrees to obtain the new user’s signature within 2 weeks. Failure to do so, will result in a suspension of the ACID.
  5. Departmental Requester submits the form by fax or campus mail to the MVS Security Administrator.
  6. When received, the MVS Security Administrator creates the ACID. ACID creations will be done every day of the week.
  7. When the ACID has been created, the ITaP clerk will send an e-mail to the Departmental Requestor stating that the ACID has been created. This e-mail will provide a summary of the day’s creations, deletes and changes.
New Mainframe Application Request/Form 183 Process:
  1. Departmental Requester obtains the Form 183 on-line.
  2. They complete the form, indicating the specific departmental system accesses needed, and sends the form to the System Owners (or designee) on campus.
  3. Form 183’s are received daily by System Owners around campus.
  4. If the ACID has been created (which it should be), System Owners (or designee) enters the access information for the requested mainframe system.
  5. System Owners (or designee) send the Form 183 (by fax or campus mail) to the MVS Security Administrator.
  6. The MVS Security Administrator adds the profile.
  7. Once the profile task has been done, the ITaP Clerk e-mails the Departmental Requester. This e-mail will be sent out daily along with the Form 182 information.
