How To Sign up for On-Line Service Request Ordering

The Telephone Department has a Web Site available for submitting requests for voice service. This can be used instead of the standard Physical Facilities Form 77. The option of using the Web Site Service is only available to authorized registered users. If your department is interested in using the web ticket for voice service requests, submit the Authorized Users Request Form.
