Hosted Survey is no longer available and has been replaced with Qualtrics Survey.

Qualtrics Survey is available at: https://www.itap.purdue.edu/tlt/qualtrics/

Qualtrics is a web-based survey tool available for use by all Purdue faculty, students and staff to support teaching and research at Purdue. Surveys can be created and distributed by anyone with a Purdue career account.
